This is a seminar at Imperial College London, open to all with registration in advance. Speaker Hadeel Mohamed, Lead Pharmacist for Education & Training at Bradford Teaching Hospitals NHS Foundation Trust, shares findings from her PhD research on how older people with polypharmacy self-manage their medicines across the hospital-to-home transition. Her talk focuses on a qualitative study of patients' experiences and behaviours around discharge, and on a person-centred approach to supporting medication self-management needs during transitions of care.
